KH-4A. All cameras operated satisfactorily.
- 1966 November 8 19:53 - KH-4A 1037 Launch Vehicle: Thorad Agena D SLV-2G. Thorad SLV-2G Agena D 507 / Agena D SS-01B 1632 Apogee: 300 km (180 mi). KH-4A. Second pan geometry mission. Higher than normal base plus fog encountered on both main camera records.
- 1967 May 9 21:50 - KH-4A 1041 Launch Vehicle: Thorad Agena D SLV-2G. Failure of the booster cut-off switch.. Thorad SLV-2G Agena D 508 / Agena D SS-01B 1634 Apogee: 791 km (491 mi). KH-4A. Due to the failure of the booster cut-off switch, the satellite went into a highly eccentric orbit. Significant image degradation.
- 1967 June 16 21:35 - KH-4A 1042 Launch Vehicle: Thorad Agena D SLV-2G. Thorad SLV-2G Agena D 509 / Agena D SS-01B 1633 Apogee: 380 km (230 mi). KH-4A. Small out-of-focus area in forward camera of 1042-1.
- 1967 August 7 21:44 - KH-4A 1043 Launch Vehicle: Thorad Agena D SLV-2G. Thorad SLV-2G Agena D 510 / Agena D 1637 Apogee: 346 km (214 mi). KH-4A. Forward camera film came out of the rails on pass 230D. Film degraded past this point.
- 1967 September 15 19:41 - KH-4B 1101 Launch Vehicle: Thorad Agena D SLV-2G. Thorad SLV-2G Agena D 512 / Agena D 1641 Apogee: 372 km (231 mi). KH-4B. First mission of the KH-4B series. Best film to date.
- 1967 November 2 21:31 - KH-4A 1044 Launch Vehicle: Thorad Agena D SLV-2G. Thorad SLV-2G Agena D 513 / Agena D 1639 Apogee: 410 km (250 mi). KH-4A. All cameras operated fine.
- 1967 December 9 22:26 - KH-4B 1102 Launch Vehicle: Thorad Agena D SLV-2G. Thorad SLV-2G Agena D 514 / Agena D 1642 Apogee: 248 km (154 mi). KH-4B. Noticeable image smear for forward camera, but best imagery to date.
- 1968 January 24 22:26 - KH-4A 1045 Launch Vehicle: Thorad Agena D SLV-2G. Thorad SLV-2G Agena D 516 / Agena D 1640 Apogee: 438 km (272 mi). KH-4A. All cameras operated satisfactorily.
- 1968 March 14 22:00 - KH-4A 1046 Launch Vehicle: Thorad Agena D SLV-2G. Thorad SLV-2G Agena D 518 / Agena D 1638 Apogee: 390 km (240 mi). KH-4A. Image quality good for 1046-1 and fair for 1046-2.
- 1968 June 20 21:46 - KH-4A 1047 Launch Vehicle: Thorad Agena D SLV-2G. Thorad SLV-2G Agena D 517 / Agena D 1645 Apogee: 402 km (249 mi). KH-4A. Out-of-focus imagery is present on both main camera records.
- 1968 September 18 21:32 - KH-4A 1048 Launch Vehicle: Thorad Agena D SLV-2G. Thorad SLV-2G Agena D 524 / Agena D 1647 Apogee: 393 km (244 mi). KH-4A. Film in the forward camera separated and camera failed on mission 1048-2, also the stellar/index camera unit failed.
